Thanks a stack Joe

I tried all of that and I am now sure I have a doorstopper/paper weight .
I remove the card & reseat it and it shows the extract/insert , status red
light, status lights green , status lights orange & then shows that the card
FAILS .

Nice when that card has to go into a campus implementation this coming
weekend !

A couple of things to try before opening a TAC case:

1.  show modules,  is the module recognized
2.  show test <mod#>,  what are the results, any failures
3.  reseat the module,  what are the console messages during this reseat
4. if all are still failures, verfiry a different slot in the chasis
