When you say the provider has checked the line, are you saying the ISP ???
The Telco's & CLEC's have a lot of expensive equipment to test these lines.
I worked for a CLEC that used Harris & Turnstone line testers from the CO
(copper termination) to the Demark.  @ the prem there are testers called the
Sunrise & Sidekick (& not all techs know how to use them).  Unless you've
had the Telco out to stress the line, I don't think you could rule it out.
Ultimately you may have a bad copper pair on the F1, F2, or inside wiring.
Also- there is a situation dealing with too many "High Cap" circuits in a
bundle.  ISDN (like DSL) is very susceptible to noise or bleed.
