Thanks Muhammad - I stumbled across that one myself as well, it is checked as 
"All" instead of "Any" and still no luck. I have also tried temporarily 
removing the mandatory role and it still doesn't kick. When I look at the 
reports it shows that the Requirement is there but it's not triggering the 
rule...

Just double check that you have selected the radio button option :

 

Clean Access Agent à  Requirements à Requirement Rules à 'All selected rules' 
succeed   instead of 'Any selected rule succeeds
