What we do is "Alternatives to Restraints" form prior to initial "Restraint evaluation".
We do the alternative form for up to 7 days or as applicable- this has documentation which proves what has been attempted prior to applying a restraint.
Then.. We do the "restraint eval"
Then Monthly we have a short form which is completed with monthly summaries on wether they continue to be appropriate/ and or reduction is needed.
