Briggs has an excellent pre-restraining assessment that includes all of
the factors that need to be evaluated. It is form CFS 3-1HH.

Subject: Restraint Assessment

Does anyone have a pre-restraining assessment or admission restraining
assessment they would be willing to share? I have attached what I
currently
use for pre-restraining. Then quarterly I use the Briggs restraint
reduction
form.

Thank you in advance,
