A leak at the heat exchanger was exactly what I suspected.  The heat
exchanger ends should come off easily with 4 nuts/bolts on each end.  The
tube bundle and deflector plates should just slide out.  If even just the
o-ring has gone bad you can get coolant leaking into the raw water.  You'd
ever see the leak.  A coolant system pressure check might prove/disprove
that a leak exsists
