Thanks Jonathan, but that's exactly what I did, and I couldn't replicate the problem in my test project, which is why I'm saying that something outside the multithreading is screwing things up somehow, but that could be anywhere in 23,000 lines of code, and I don't know what to look for. This is very much plain vanilla obj-c and core graphics code - there are no fancy behind the scenes tricks or anything that I am doing.
