The definition is pretty clear in my opinion. 'append' means insert rule at the end of the chain, 'insert' at any position defined by the index. no?
No index in the story here. Moreover we don't handle rule numbers in our code. append put the rule at the and of the list, insert puts the rules at the beginning of the list.
