Once you've setup fstab:

mkinitrd /boot/initrd-2.4.18-6mdk.img 2.4.18-6mdk --with=ext3

I just checked the script and it checks /proc/mounts, rather than fstab,
so to be sure, you should probably call for ext3 specifically.
