Ok, with DEFAULT set to moderate I get this:

$ cat courier-list-default.12830 | /usr/bin/couriermlm ctlmsg /home/bmw- mcs/couriermlm/
Invalid moderation message.

Looking at the original message you posted, it does not have the ==CUT HERE== delimiters.

Put two lines:

==CUT HERE==
==CUT HERE==

Before the tokens, and see if it works.
