This sounds very much like a sage old L4D2 matchmaking bug, where you would get "7/8 syndrome" and the 8th player slot was never usable on the server, or even returning to the lobby. The only solution was to destroy the lobby and re-lobby again. 8/8 could then connect and people join / leave as normal.
