Thanks. That is helpful. I think the problem is the configuration you are giving debian-installer. As you don't have path_map configured you need to specify the url of the upstream mirror as well. I think the host setting is fine (http://apt-cacher-ip:3142) But the directory should be the host/directory of your upstream mirror: eg ftp.debian.org/debian Alternatively, set something like path_map debian ftp.debian.org/debian in the config.
