I've been running under the assumption that STARTSWITH works on the beginning of the entire body of the scope (HEADERS, SUBJECT, BODY). Are you saying that STARTSWITH is smarter than that, that it looks at the start of each line? If so, is this true for all scopes?
No, it only looks at the beginning, it does not check every line. So it should actually be:

HEADERS -5 CONTAINS In-Reply-To:
