I had an idea. What about some sort of combination dropdown/button that would allow you to choose files or folders (or some future goodies like regular expressions)? That way, still just two controls... add-something and remove.
Alternatively, maybe redo the idea of a list with attached controls and do something closer to evolution or thunderbird's mail filter dialog where you add rules and click a + button to get a new rule line. I should probably ask a designer, if I can grab one.
