Steps I took to check in Internet explorer:
1. Go to "File" -> "Page Setup"
2. Specify "This is a test" in Footer
3. All print previews after step 2 had "This is a test" in footer portion
(Bottom left) of each page.
So, if the applicant name is "Steve", just specify the same text ("Steve") in
Footer (in Page setup) portion when a web page is loaded.

Thanks for your reply. What we need to do is set a placeholder in the footer
that is automatically filled with the name of the person whose form you are
printing. Is that what you mean by " .... specify a custom text as footer
....."? I think my poor explanation may have misled you.
