We actually pend all orders from _first_time_ buyers, lookup the IP using 
visual traceroute and make sure that the order was placed from somewhere 
near the actual billing address. If there is anything suspicious, we call 
and ask for both the issuing financial institution and the cvv2 on the card. 
We only trust a phone number that matches what the issuing financial 
institution has on file. If it does not match, we call and ask for the 
correct information first... In most cases, it only takes a couple of 
minutes to do this since we usually don't have to call. Since we started 
with this policy, we have had no chargebacks... 

Granted, we're not doing thousands a month like some of you, which make this 
undoable.
