As far as I see there are two possible reasons for this scenario:
1) You have not restared the OOo or a you left the quickstarter active
2) The dictionary list contains now two entries for the same language
   e.g.
   THES de DE th_de_DE_v2
   THES de DE th_de_some_other
   In this case I think it was the last entry that wins.
   Thus if the entry for the latest downloaded dictionary is not the
   last it would be a bug.

A variant of 2) is that you (AFAIK) can not use the Danish regular spell
check dictionary along with the Danish medical dictionary at the same
time. That is unless there were some improvements in this respect in the
last meanwhile...
