well In my experience accessibility always comes as an afterthought

that is  someone reads an article on it and pipes up  at a very late stage
of development and then going back to impliment it becomes a time consuming
task .. so its normally left out

 what needs to happen is specs need to take accessability issues into
account
Clients need to be made aware that this IS an important issue from the get
go

 anyway
