I had a look at this, but it seems you worked with my (wrong) work? I mention
this because the code in your zip file creates the image in the n0 part, which
was wrong and fixed in PDFBOX-3817, and createBackgroundLayerForm() is missing,
see my comment from June 5th 2017. Please try to work with the trunk and modify
that one, while getting inspired by the previous works. I'd also like a PDF
produced from the code.
