MediaWiki is released on a rolling quarterly release schedule. If you keep up with the rolling release, the impact to doing the upgrade is minimal. In between the quarterly release are all the betas and release candidates (not recommended for live Wikis).
I recommend keeping up to date if time permits, so we can benefit from the most recent Wiki updates and enhancements.
I do upgrade some Wikis regularly, so if you need some assistance, I might be able to help, depending on my time.
