webbase password retrival

Hello     I was wondering there is any issue someone know of the webbased password retrieval that a lot of us use. For some reason all requests have stopped working with the only result that being "The Password has NOT been sent to the Administrative Contact"    It was working fine a few weeks ago but started getting a lot of reports of error did the last code change possible  change something as I have not upgraded yet.
