I didn't see the duplicated id anywhere, Jorn, but I did go ahead and change the name of the form to MC_Form just in case of a duplication.
That didn't help. The plugin is still unresponsive. And I'm showing the version of the plug-in is v1.0 beta 1 in Firebug.
