Sounds like you've already done much of the troubleshooting, so I'm not left with many good suggestions.
Have you checked to see if the server clock has noticeable drift from correct time? Have you noticed any difference in behavior on transcoded vs non-transcoded tracks? If you set up a test server on a regular PC and do the same thing, does the problem go away? If the answer to the last question is yes, then it's something about the NAS.
