Answering myself.

Someone over on the Skype forums has just pointed out that Hangouts has a 
desktop App and I immediately recalled that I'd tried it some time ago.  I 
just tried it again and it works quite well, but video performance is still 
rubbish.

I just did some measurements on my internal network and even using my hi-res 
camera and with the bandwdth slider set to max, Hangouts only consume 220 kbps 
on the uplink (compared to approx 480 kbps for Skype).  As before the video is 
grainy and the lip-sync awful.

So it would work fine with my broadband capability, but the quality would be 
unacceptable for talking to my Mum.
