Here's a new version that has this fixed, albeit with a specific hack rather
than a solution for the problem that custom keycode mappings are no longer
supported.

I have to think a little about how to integrate custom mappings into the
current host code. Since DQSDTools and the host are in different DLLs, we
can't use the existing code at all.

I'm thinking of adding a window.external.keyCodeMappings object, perhaps? Or
just a diversified interface with any and all custom features we can think
of: window.external.mapKeyCode, window.external.openWindow, etc.

How do you all feel about adding features to window.external? Can you see
any problems with it?
