I'm the guy that was asking about the connection to the third port on 
the mini-dock last year, and with the little grey Apple DAA it works 
fine. BUT....the wires between the DAA and the phone system have to 
be just so - which explains the taped cross connections of cable 
hanging from my mini-dock! You may need to experiment with which of 
the four wires from the DAA goes to which on the telecom side.

I'm not a hardware expert, so I don't know whether these would be of 
a great help,
but my limited knowledge makes me think they might be useful.

Obviously the best solution would be for someone on the list who has 
the elusive DAA to open it up and tell the rest of us what's inside 
(slugs and snails and puppy dog tails?)

Yes, I've noticed this. And even downloads to a Duo280 aren't too bad 
for occasional use.
