I used a program about 5 years back called AnyLabel.  I did a quick search and 
see it's still around.  Aside from all the included templates that were 
included, you can modify them or create your own.  I'm sure you'll find the 
template you need.  If not, modify one that's close.
   
  It was very easy to feed it a file from an application like Access.  There 
were many other nice features like knowing what position it was at when the 
last label printed (if you're using a sheet of lables for maybe addresses) or 
remembering the last number used for a value that increments.  Best thing, the 
price was reasonable.  I believe there is a free dowload, so check it out.
