the process to a well designed *anything* is to sketch on paper a lot of ideas 
before pushing final pixels.

Depends on when you went to design school. Before or after the great digital convergence.

A lot of designers only work with computers, or their preliminary sketches are little more than doodles.

All these concepts are so far along the path that it makes it hard to make 
sweeping changes, because the difficulty of the actual changes to the image and 
the amount of effort to overcome the inertia needed to make sweeping changes.

It's all digital. Much easier to change than a drawing. Want to move the knife? Pick it up and move it. Viola, new orientation.

Want to change its shape? drag some handlers, new shape.
