I tested PowerMail, Thunderbird and around a dozen other email
clients. I couldn't get any of them to play nicely. Finally I opted
for Apple Mail, having rejected it first time around.
Multiple accounts can be created by holding [alt] while clicking [+]
then entering whatever you like in the relevant fields (without the
darn "wizard" complaining that it can't connect!)
Incoming message subject lines can be coloured using "Rules" or by
using key presses (by installing Mail "Act-On").

In fact Mail works great for me. My only two complaints are its
occasional sluggishness (but when you have as many accounts and
stored messages as I have, it's hardly surprising) and the lack of a
script to let me change the subject line. There's a workaround but
it's not ideal.
But I'm very happy with it and, of course, it's compatible with all
the other "iLife" software so it's easy to (for example) email photos
from iPhoto.
