Dear Kyle, I suppose that here is not in discussion the INSULATION RESISTANCE, I think (as far as I know, Clause 16 from "IEC335-1, Safety of household and similar electrical appliances, Part 1: General requirements", deals with the ELECTRIC STRENGTH and LEAKAGE CURRENT...)
The Table 5 of the Standard IEC335-1 gives the values of the TEST VOLTAGE and the POINT OF APPLICATION. Compared with IEC 950, within IEC335-1 you may find the fact that the required TEST VOLTAGE is higher than within IEC950 [e.g. between LIVE parts and ACCESSIBLE PARTS IEC 335-1 is asking for 3750Vac/1min and IEC950(IEC60950) is asking only for 3000Vac/1min...I suppose that here was the difference and ...the failure... Unfortunately I was not able to find the Clause 5.3.101 within the copy which I have...and as well I did not find the Japanese deviations neither under the country notes, nor under the legal requirements... A remark of "Not evaluated as part of this investigation" also appears. The text of the Deviation + Test reads: "Add: The insulation resistance shall comply with the requirements of Clauses 16.3 of IEC Publication 335-1 (1976) Second Edition." I use UL1950 2nd and 3Rd ed. and I cannot find this 5.3.101 reference in the latest copy of the standard, much less IEC Pub 335-1.
