When I had this problem, it was a set screw on the encoder shaft
that I forgot to tighten.  It would almost track, but a bit of error
would build up after many turns.

If your encoder is differential, make sure the 7i33 is jumpered for
differential mode.  If it's not, make sure the 7i33 ISN'T jumpered for
differential mode.  Also check your grounds and as Jeff says, check
the mechanical stuff, even if you already checked it once!
