Thanks to all who helped with this problem;

My limitation was a lack of experience with EMC, plus:
There is documentation on how to setup the .ini file.
But I could not find documentation or examples how to
setup the .hal file for that configuration. It wasn't working
so I assumed something was missing. I have been poking
at this problem one day at a time for months.

My first call for help, got a response from someone
having the same problem and he had given up and gone
with the switches. This reinforced a bad assumption
for me. I wasn't alone and it may be the EMC system.

My last call for help got several responses.
1- Upgrade to EMC 2.4.2. (No change).
2- Better documentation for .ini setup. (No change).
3- Jon said it has been working for years and had an
    example .hal setup. It was the first setup I had tried.
    OOPS.

The problem was;
I had wired the X & Z Encoder Index input to the
Motenc card like all the other Encoders, + to + and - to -.
My X & Z Index Pulses were negative active, like all
the single bit DIO into EMC. However EMC is expecting
a plus active level on the Index Pulses. This allowed Emc
to see a plus level immediately after the Limit/Home switch
relsase. Once this was realized, I simply swapping the leads
+ to - and - to + and all is well.

I assumed all encoders were the same.
As a supervisor I have said "never assume".
"Just Look at the word" (ass-u-me).
Wish I had followed my advice.
