I think, if I had the horsepower, that I'd use a fly cutter with a jack 
under one edge of the 4th axis to cut the tops of the ramps.  But I don't 
fool myself as to the horsepower available in my toy.  For me it would 
take longer but a 1/4" 2 flute could do the ramps with the jack under the 
4th axis to get the tilt.  Or it could even be done laying flat with the 
appropriate gcode as a path up and down the ramp at millimeter spacing 
would not leave an unusable roughness to the finish there.  Under those 
conditions, rather than the 4th axis, I think I would rotate the co-
ordinate map to do each 'hook' & re-use the same code.  Its far tighter 
than my 4th axis anyway.  Machining the spanner wrench notch would be a 
stand it up in a vice operation, NBD.  I can't see the bore, so I have no 
clue but I can't believe that would be a problem for you.

Given some 5/8 or 3/4" stock of suitable width, I think I could make a 
serviceable copy on my machine.  But ATM my heaviest stock is too thick at  
1" x 2" wide, not wide enough.
