Both. Made the pattern, cast it and machined it.  In this case it's old Honda 
Intake Manifolds.   Most of the time I use my CNC router to make the patterns.  
In this case the motor plate buttons were taper turned on my Gingery Lathe 
using the ELS to give me 3 degree draft.

The shaft spacers are turned from a casting sprue.  Turned down to 25mm.  
Drilled to 1/2" and then parts are bored to 17mm depending on where the spacer 
sits.  Depths of holes are controlled by the ELS after touching off the face.  
I also had to enlarge the hole in a steel washer so the original lock washer 
wouldn't mangle the aluminium spacer.  

As always there are things I would do differently.

For example I thought I'd installed the 18T pulley but it was the 24T.  When I 
tried a Y1.000 move it went further than it was supposed to.  A Y0.2 should 
have made it do one revolution of the lead screw  (5TPI).  I wondered why the 
belt was so tight.  One I replaced the pulley with the correct one I appear to 
be matching my Shumatech DRO within 0.001".  That's with the backlash in the 
ini file set to 0.008.

I have yet to try moving to a point from either direction to see if that all 
works correctly.

Even with the 18T pulley it's still scary fast at 150 TPI.

Oh and the Knee castings are also made with a two part pattern with room for 
extra angular contact bearings with a custom spacer to move the bevel gear so 
the knee backlash is close to 0.  Ideally I'd rather drive the knee screw 
directly but this si fine for now.
