Well all mailboxes already have prohibit send & receive limit set.
And there are about 120 mailboxes out there, which I don't see them all
in ESM. So does that mean I have to go thru them one by one to figure
out which one is the culprit? That makes it a bit tedious especially
since I have to do most of it remotely.

I told you:  "See if one mailbox in particular is growing."

If you find that one is, you can set a prohibit send and receive limit
on
it, cycle the Information Store, and that should kill the mail loop dead
in
its tracks.

Mailbox store has been growing rapidly with no mailbox activity.
Performed offline defrag and brought it down from 15GB to about 12GB.
In just 2 days store has grown by 250MB without any mailbox activity.
While the mailbox store is about 12GB, the total of priv.edb & pub.edb
are
only about 8GB. The priv1.stm is about 4GB.
There are mailbox limits and Backups are running fine. 
What could be the cause of this and how can it be stopped/fixed?
