I like to to keep the load on my browser in the daily use at a minimum. So Firebug should be always off by default (and visualized to the user by a grey roach). When I activate it to check a site I normally deactivate it afterwards. No need for site rules IMO.
With always off as default there should be only one option that affects this: If I forget to deactivate Firebug I would expect it to start deactivated again.
