At http://sourceforge.net/projects/fluidsynth/ there's a ticket link near the top. You need a sourceforge account (and to be logged in) to see the "create ticket" button.
As to etiquette, fluidsynth doesn't have dedicated patch tickets so, at the very least, starting the patch subject with "[PATCH]" is probably good form.
