Shuttle have a pretty good thing going with their cases, though
the shape is a little inconvenient.  They use a liquid-cooling
system to drain heat from the CPU.

Mine -- with a 1.6GHz Celeron -- was pretty quiet until I made the
mistake of putting a big disk in it.  I've since opted for a bigger
case and more standard parts, with long cables so it can go in a
room where the noise doesn't matter.  Works beautifully now,
though I use WebVCR+ for the recording side.
