Heya. How do you mean "lost"? Does it stay for a while and then vanish on subsequent boots, or does it vanish immediately after reinstalling OS X?
It stays for a variable amount of use of OSX --- If I do some WEB surfing, something gets ZAPPED somewhere, and after a 'reboot' or 'shutdown', osx will not even start to load --- so no I haven't been far enough into things to figure 'booting without extensions', or working on 'permissions' would have any effect. I don't even get to any sort of video response, even just having the thing try to turn on!.
When I say I wait for 'no activity', I have very good ears, almost to the point where a 'stethescope' would be 'overkill'.
For what it's worth, my bootup times with my beige G3 aren't terrifically fast and it takes usually about 30 seconds just to get a signal to the monitor. (This is with an ATI Xclaim BTW, not built-in video.)
Have you done the usual dances? Shift on reboot? Verified/repaired permissions?

A little more info
I added an ATTO SCSI card and a couple of HDs, one having OSX , the other having 'stuff' and OS9.1.
I could then, usinf 'System Disk' get to THAT copy of OSX, but could NOT find a way to get to the OSX on the ATA drive.
Latest try was to switch the IDE cables in the Beige. I'm on OS9.1 on that drive now, ATTO card and SCSI drives removed, (It wasn't getting me to OSX on the IDE drive. So wasn't useful.)
