My thought was that he did not want to take the rehab time off to have that cyst removed. Another possible issue is that the procedure could be rather risky and he may not have (or has) not wanted to take the risk.

Following the surgery he will rehab whatever heart issues he may have. If his heart problems are really stress related, I suspect that he will significantly increase his exercise regiment to get the stress under control.
