No.  Not with Packet 8 anyway.  You can get a local number in the 225 
area code.

Any router.  They do not require a special router.

Behind the router in my case.  I am considering getting a second IP 
from Cox so that I don't have to deal with the routing issues.  I am 
using a hardware router and it does not offer me much flexibility.
