Hi Phil,
I believe that what I ended up with was creating the ImageMosaic files more
or less by hand (I wrote a program to do it), then pointing the ImageMosaic
to the shp file in the directory, in the Connection Parameters box  of the
Add New Store dialog, I put:

*file:E:\GeoServer_Coverages\OK_City_2008_One_Foot\mosaic.shp*

This local directory on E: has the following files:

mosaic.dbf  - contains shp dbf records that point to the UNC file, for
example:

*
\\Ogisrv103\ogisrv103_F\GeoServer_Coverages\OK_City_2008_One_Foot\QQTile1919.tif
*

mosaic.shp - contains the shape files for the images
mosaic.shx - the shapefile index
mosaic.prj  - contains the wkt prj info:

GEOGCS["WGS 84",DATUM["WGS_1984","SPHEROID["WGS
84",6378137,298.2572235630016,AUTHORITY["EPSG","7030"]],AUTHORITY["EPSG","6326"]],PRIMEM["Greenwich",0],UNIT["degree",0.0174532925199433],AUTHORITY["EPSG","4326"]]

and mosaic.properties - here it is:

(my files have overviews)

I just tested this in a very recent nightly build of trunk, and it works.

Finally, the error you received is the generic error the ImageMosaic emits
whenever it hits a problem - not very descriptive!
