Try
"\\rightalign{\bf 1. }FirstWord"
(The \rightalign macro is introduced in the Lilypond TeX header file,
there is an equivalent macro in TeX called \llap. Both put the text
to the left of the "insertion point".)
The spacing calculation will still not be perfect, though, maybe
everything that's included in brackets should count as zero length
by Lilypond.
