Unless the shot was still compacted together in one mass, say within a two yards or so of the muzzle of the shotgun, it's not going to do much to someone wearing a decent vest other than cause one hell of a wound and maybe a cracked rib. Even though the total energy of the payload, in this case, the shot, may be 1500 ft/lbs or thereabouts, the area it covers grows and the individual impacts are much less than this total would lead one to believe.

Slugs are a different story altogether.

I'm pretty sure this is the way it is supposed to be handled, and looking
at it I think it is pretty reasonable.  A point blank shotgun blast to
kevlar is not going to be pleasant and has every probability of being
fatal

Again, it would depend on the load of the shotgun, the range, and the vest involved. Against someone with no armor, I would prefer to say potentially fatal, not probably fatal as there's too many cases where people have taken multiple hits of 12ga 00 loads and not DRT (died/dead right there).

Again, this is real world and the game may not model it as accurately.
