Rick, Are you working in Vista or xp?
I'm having this kind of issue routinely and frequently in vista, but very seldom in xp. When it happens to me I often get no error message at all, it simply stops speaking. Sometimes I'm able to restart window eyes, and sometimes I have to reboot as I keep hearing the error sound from the dialog that's telling me that window eyes is already running. Yes, I'd say switching between applications triggers it more frequently than anything else.
