Another tip regarding reading form fields is to try using the Window-Eyes
speak field name and field data hotkeys which are Control-Shift-N and
Control-Shift-D in the default keyboard layout.  These commands can be very
helpful when trying to identify the name/label of the form field you are in
or the data that exists in the form field.  For example, if you are in a
phone number edit box after typing your name, the field name hotkey would
speak "phone number" and the data hotkey would announce what text has been
typed in the edit box.  Have you ever pressed Tab and then Shift-Tab to
figure out where you are in a dialog box or web page form?  If so, you could
use the speak field name and field data hotkeys instead to identify where
you are focused without having to move the cursor.  These commands do not
work in all types of forms but work well on the web and in dialog boxes.

Assuming the form was marked up correctly, the speak summary command 

(Control-Shift-S) should do the trick. If not, however, you might try 

using the previous and next line commands (Alt-Comma and Alt-Period 

respectively) to get some context as to the form's layout. If that isn't 

enough, then the form's author may need to make the document accessible.
