That's an Iranian national standard, ISIRI 2901:1994. The standard is 
named something like
"Keyboard layout for Farsi: Characters for Computer", but also works for
writing Arabic also (which is usually needed as quotes in Persian texts).

Most keyboards come with a very archaic ISIRI 2901:1989 layout printed on
the keys, which is not implememtable easily, since it is a two-symbol 
keyboard: you type "g" for initial and medial "Lam" and "shift-G" for 
final and isolated "Lam".
