Radoslav, I agree that people are the problem not the systems, but remember that systems are developed by humans. both, developers & sysprogs do mistakes. sorry to report that they (mistakes) are quit common.
and for the product, it is VERY common. It speedup data retrieval form racf by reading the database. as it executes in the user region, the user must have at least READ access to the database. The vendor has some other products that are based on this product to retrieve or update RACF. the result is that even read-only auditors has access to the database. BTW, the product do have a server only mode, by the vendor claims this is not supported by all products that are based on.
