Jeff, Sense byte 6 = x'25' indicates the drive is in read-only format. That appears to be symptomatic of a media format/drive type misconfiguration to me.

Given that it's a problem only ever reported here once - and that a long time ago - and that there must be several folk successfully using these drives every day, I'm leaning towards some kind of configuration problem but I'm not seeing it.
