I had this happen on numerous occasions in the past. It seems to have gotten better, but it is so random (like 1 in two weeks, with an average of 150,000 messages during that time), that I haven't put much effort into solving it. I think I turned it into Ipswitch in the past, but they gave me the standard, change the templates back to stock answer, that I pretty much have given up on them for support.
